Application Process
Departments within The Regional Medical Center at Memphis (The MED) with programs or proposals consistent with the priorities of The MED Foundation may apply for a grant. Projects must be brought to The MED Foundation through an employee of The MED.
An application form is required and can be obtained by clicking here. All proposals must include the following information:
- The department’s full legal name, complete mailing address, phone number, fax number, contact person's name and e-mail address.
- A list of key individuals involved in the project.
- A statement describing the specific purpose of the grant requested, including how the project meets the priorities of The MED Foundation and The MED. For example: what is the community/hospital need to be addressed; description of the target population if applicable; timeframe for activities; number of people served; description of how this project enhances the quality of life for patients, or how it enhances work environment for employees, etc.
- A statement of the amount of money requested and over what period of time.
- A plan for how the program or services will be sustained after the conclusion of funding from The MED Foundation.
- A plan for measuring results and reporting periodic progress, as well as for a final evaluation.
- Financial information/ budget for the proposed project or item, including whether or not this project was included in the annual budget for the department. If no, why not? And if rejected, why?
- If requesting funding for equipment, a copy of the quote received through The MED’s purchasing department must be attached.
- All proposals must be approved by the requesting department’s vice president before submitting the application.
